By signing below the participant agrees to the membership agreement to pay on time each month for the full term (1 year /12 months) on eft billing. If the participant wishes to cancel their membership at Hard Knocks Muay Thai & MMA with less than 50% of The remaining balance, a $150 Cancelation Fee May be applied. A written letter emailed to hardknocksboston@gmail.com must be submitted to Hard Knocks a 30 day Prior notice is required to process your request and must be done by email only.

Release From Liability / Hold Harmless

In exchange for participation in the activities of Kickboxing, Muay Thai ,MMA, Boxing and other combat sports/martial arts/exhibition/completion Hard Knocks Muay Thai & MMA and or use of the property ,facilities and services of Hard Knocks Gym I agree for myself and ( if applicable) for the members of my family and or to I am guardian to the following.

1. ASSUMPTION OF RISK
I am aware and understand that, in general martial arts is an intrinsically dangerous and hazardous activity and in particular, the instruction, techniques and methods of Muay Thai, MMA and Hard Knocks I shall be learning and Participating in all such activities as a member of Hard Knocks. I acknowledge herein that participating activities contemplating by this document involve an extremely dangerous and hazardous enterprise. I am voluntarily and of my own free will submitting this application for membership, becoming a member and learning and participating in the Hard Knocks programs with full knowledge and understanding of the serious danger and hazards involved .I hereby consent and agree to any physical contacts to me by my instructors, fellow students or others in furtherance of training, practice or demonstration/completion. I hereby consent and agree to completely accept alone any and all risk of injury, death and hold harmless, Hard Knocks its successors, it’s instructors/trainer, agents and employees from any liability whatsoever arising out of any activity relating to instruction, training, practice, or demonstration/competition,(on or off premises) I acknowledge, confirm and verify the forgoing by placing my signature hereto:

2. HEALTH ADVISORY AND PHYSICAL CONDITION
I hereby understand and acknowledge that the performance, practice and utilization of the methods of hard knocks involve extremely strenuous physical activity, and that I have been advised to consult a physician before commencing and undertaking any activity, contemplated by this application/agreement. I hereby represent that, to the best of my knowledge, I am in good physical health and condition, sufficient to undertake the activities and practices constituting the methods of Hard Knocks. If the condition of my health changes during the time of existence of this agreement, I will immediately notify my instructor, trainer and the owner of Hard Knocks Muay Thai & MMA.

3. RELEASE
In consideration of being accepted as a member, student, participant, instruction or any demonstration/competition in any and all of the activities conducted or sponsored in whole or in part by Hard Knocks. I hereby consent and agree to observe and obey all posted rules, regulations and warnings, and further agree to follow any oral instructions or directions given by Hard Knocks, or the employees, representatives of Hard Knocks. I recognize that there are certain inherent risks associated with the above described activity and assume full responsibility for personal injury to myself and (if applicable) my family members and or to whom I am guardian of , and further release and discharge Hard Knocks for injury, loss or damage arising out of me or my family on or presence upon the facilities of Hard Knocks whether caused by the fault of myself, my family and or to whom I am guardianship Hard Knocks or other third parties. I agree to indemnify and defend Hard Knocks against claims,causes of action, damages, judgment, cost, or expenses, including medical bills, attorney fees and other litigation coasts, which may arise from me or my family’s use or presence upon the facilities of Hard Knocks. I agree to pay for all damages to the facilities and equipment of Hard Knocks by me or my family’s negligent, reckless or willful actions. Any legal or equitable claim that arise from participation in the above shall be resolved under Massachusetts Law.

I have read the above document and understand it. I further understand that by signing this release I voluntarily surrender certain legal rights.
